TOURNAMENT ENTRY PROCESS

1. Print and mail entry form below, include your check, this is the only way to reserve a spot.

2. Teams must register online for USSSA registration number at www.usssa.com. First Pitch will NOT register your team. The registration number should be included on the tournament entry form, but can be submitted at a later date.

3. The First Pitch Baseball Team Roster and Waiver form below is required at check-in one-hour before your first game in Champaign. Print roster/waiver form, it must be signed by a parent for each player and presented to our site-supervisor at check-in.

4. USSSA requires all teams register your team roster online at www.usssa.com. This is required for all USSSA tournaments. Your team will not be allowed to start tournament play in Champaign until this is completed. Click here for printable, step-by-step instructions. Print a copy of the roster and give to our site-supervisor at check-in. No parent signature is required on this roster.
